Orlando Bloom has been known for his stiff acting and chocolate roles. With this role of a psychotic doctor, he breaks that mold. He plays the young doctor Martin Blake, very correct on the outside, with all the right motives to be a doctor, but is a secretly insecure, ambitious sociopath. He starts out badly with patients and superiors, but gets a chance at redemption when beautiful young Diane (Riley Keough) is admitted. She’s beautiful, trusting and vulnerable. Things get out of hand fast when he falls for her and makes unethical decisions. His homicidal tendencies show up when an orderly, Jimmy, (Michael Pena) finds this out and starts to blackmail him for drugs.

The movie is detached from the psychosis, as in Blake’s head, what he’s doing is justified. The viewer initially identifies with the need to excel, but that slowly turns to horror on seeing the depths of his obsession. Director Lance Daly and scriptwriter and writer John Enbom forge the tension well; it is almost palpable in the scenes where we see Blake furtively running around, stealing chemicals and lacing Riley and Jimmy’s drugs.
